Deadline of Transferring PhilHealth YAKAP Clinic is December 26 at PhilHealth Offices

The Philippine Health Insurance Corporation (PhilHealth) reminds all members who wish to transfer their PhilHealth YAKAP Clinic that they may visit any PhilHealth Local Health Insurance Office (LHIO) until Friday, December 26, 2025. This ensures uninterrupted access to benefits under the PhilHealth YAKAP Program.

Members or their dependents may transfer to a different YAKAP Clinic, whether or not a First Patient Encounter (FPE) has already taken place. They must go to the nearest LHIO, submit a completed Primary Care Selection Form (PCSF), and undergo verification through the PhilHealth Check Utility (PCU). PhilHealth will notify members by email regarding the transfer details, which will take effect upon approval starting January 1, 2026.

The PhilHealth Action Center at (02) 866-225-88 will also accept transfer requests until December 31, 2025.

PhilHealth allows requests for transfer on behalf of the member, provided the representative presents an authorization letter, one valid government-issued ID (or acceptable alternative documents), and a duly accomplished PhilHealth Member Registration Form (PMRF) or PhilHealth Claim Form (PCF), as applicable.

PhilHealth reminds members that those who have not yet completed their FPE may request a transfer at any time. Once approved, the transfer takes effect immediately. For members who have already availed of their FPE, transfer requests are allowed only during the fourth quarter of the year (October to December).

Members may view the directory of YAKAP Clinics at www.philhealth.gov.ph to view facilities near their location to guide their selection. The list is constantly updated to include newly accredited YAKAP Clinics and updated contact details.

PhilHealth President and CEO, Dr. Edwin M. Mercado emphasized, “Ito ang direktiba ng Pangulong Ferdinand R. Marcos Jr. – na magamit at mailapit ang primary care benefits sa lahat ng Filipino. Ang bawat miyembro ay may kalayaang pumili ng YAKAP Clinic upang mas madaling magamit ang benepisyo ng PhilHealth YAKAP. Maaaring ito ay malapit sa kanilang tahanan o trabaho para masiguro ang kaayusan at kalusugan ng buong pamilya”, he said.

PhilHealth YAKAP offers free health services, including regular check-ups, laboratory tests, cancer screenings and medicines under PhilHealth GAMOT (Guaranteed Accessible Medications for Outpatient Treatment).
